Beyond an answer log

Keep the source basis and review context together.

A transcript of an answer is not enough to show why the answer was produced. Assurance work may also need the authoritative policy version, cited location, retrieval route, warnings, selected scope, model details where AI was used and the human follow-up that occurred.

PolicyOps keeps those elements structurally distinct. Primary evidence is not conflated with related context. Metadata answers identify their source fields. Missing trace or integrity information on legacy records is made explicit rather than silently treated as complete.

The workspace, question, answer route, timestamp and available user or request context establish what was being considered.

Retrieval Trace information and source intent remain inspectable. Model and prompt details are retained where configured AI contributed.
